Liquids from the High Temperature Separator (D4), Low Temperature Separator (D5) and HDT Low Temperature Separator (D3) are fed to the HP Stripper (T4) for separation of naphtha and lighter material from the dewaxed oil stream. Pressure letdown, heat in the high temperature liquid, and MP stripping steam to T4 bottom provide for flashing and stripping of the lighter material in the column. The overhead vapor from T4 is condensed in the LP Stripper Overhead Condenser (E3). Effluent from E3 is phase separated in the LP Stripper Overhead Accumulator (D11). Vapor from D11 is used as fuel gas. A portion of the unstabilized naphtha from D11 bottom is pumped by the LP Stripper Overhead Pump (P2 A/B) back to the tower as reflux. The net unstabilized naphtha product is sent to battery limits for further processing.
The bottoms product from T4 is sent to the Vacuum Fractionator Charge Heater (F3) to meet the required inlet temperature for proper fractionation. In the Vacuum Fractionator (T5), medium distillate and various lube cuts are fractionated. Distillation is carried out under vacuum to avoid thermal cracking and degradation of the dewaxed base stock.
The Vacuum Fractionator overheads proceed to the Vacuum Fractionator Vacuum System (X2). The Vacuum System package will include a steam ejector system with pre-, inter-, and after-condensers. The Vacuum System condensate is phase separated in the Vacuum Fractionator Overhead Accumulator (D12). The condensed hydrocarbons (slop oil) are recycled by the Vacuum Fractionator Slop Oil Pump (P8 A/B) to the LP Stripper after being heated by the Heavy Lube Product in the Slop/Heavy Lube Exchanger (E22). The vapors from D12 are combusted in the Vacuum Fractionator Charge Heater (F3). The sour water decanted from the accumulator is sent to battery limits.
A top pumparound condenses most of the vapor produced in T5. The pumparound liquid is withdrawn and pumped by the Pumparound Pump (P4 A/B). Part of the liquid is returned directly to T5 as reflux, with the remainder being sent to the Vacuum Fractionator Pumparound Cooler (E4) and Trim Cooler (E18). The cooled pumparound stream is returned back to T5, and the net condensed distillate is sent to battery limits as medium distillate product.
The Light Lube product, withdrawn from the Vacuum Fractionator (T5), is stripped in a Light Lube Stripper (T7) and dried in the Light Lube Dryer (T8). The Light Lube product is pumped by the Light Lube Product Pump (P5 A/B) to the Light Lube Product Cooler (E5) for cooling before it is sent to Light Lube Product storage.

В вакуумной фракционирующей колонне (T5) происходит фракционирование среднего дистиллята и различных масляных фракций. Перегонка осуществляется в вакууме во избежание термического крекинга и ухудшения качества депарафинизированного базового масла.
Верхние продукты вакуумной фракционирующей колонны направляются в вакуумную систему вакуумной фракционирующей колонны (X2). Комплектная вакуумная система будет включать систему пароэжекторов с предварительным, промежуточным и вторичным конденсаторами. Конденсат из вакуумной системы разделяется на фазы в сборной емкости верхнего потока вакуумной фракционирующей колонны (D12). После подогрева теплом тяжелого масляного продукта в теплообменнике некондиционного продукта/тяжелого масла (Е22), сконденсировавшиеся углеводороды (некондиционный продукт) направляются насосом некондиционной продукции вакуумной фракционирующей колонны (P8 A/B) на рецикл в отпарную колонну НД. Пары из D12 сжигаются в печи сырья вакуумной фракционирующей колонны (F3). Кислая вода, слитая из сборной емкости, направляется на границу установки.
В контуре верхнего циркуляционного орошения конденсируется основная часть паров, образующихся в Т5. Жидкость в контуре циркуляционного орошения отбирается и откачивается насосом циркуляционного орошения (P4 A/B). Некоторая часть жидкости возвращается непосредственно в T5 в виде флегмы, в то время как остальная часть направляется в холодильник циркуляционного орошения вакуумной фракционирующей колонны (E4) и концевой холодильник (E18). Охлажденный поток циркуляционного орошения возвращается в T5, а нетто-объем сконденсировавшегося дистиллята направляется на границу установки в качестве продукта среднего дистиллята.
Легкий масляный продукт, отбираемый из вакуумной фракционирующей колонны (T5), отпаривается в отпарной колонне легкого масла (T7) и осушается в колонне вакуумной сушки легкого масла (T8). Легкий масляный продукт подается насосом легкого масляного продукта (P5 A/B) в холодильник легкого масляного продукта (E5) для охлаждения перед отправкой в парк легкого масляного продукта.

Source text - French Les aciers inoxydables ont une grande résistance à la corrosion du fait de leur aptitude à demeurer ‘‘passifs’’ en présence de nombreux milieux.
A l'état ‘‘passif’’, ils sont recouverts d'une mince couche protectrice d'oxyde de chrome, invisible et ‘‘étanche’’ à l'air, de grande stabilité chimique, appelée couche passive.
Cette couche se ‘‘cicatrise’’ spontanément à l'air lorsqu'elle est faiblement endommagée, toutefois l'acier inoxydable n'est pas parfaitement inaltérable.
Il peut être contaminé lorsqu'il est en contact avec des agents “corrosifs’’ ou lors qu'il subit des agressions mécaniques (chocs, rayures, projections de particules ferreuses, lors des travaux de soudage, meulage, tronçonnage d'acier ordinaire à proximité…).
Les principaux agents corrosifs sont les ions halogènes (Chlorures - Fluorures - Iodures - Bromures) et les dérivés du Soufre.
Leur limite de concentration est difficile à préciser car nombreux sont les paramètres qui influent dans les phénomènes de corrosion (voir page suivante).
A titre d'exemple, les aciéristes considèrent qu'il y a risque de corrosion de l'inox 316L - Norme 1.4404, en contact avec une solution contenant plus de 150 ppm de chlorures, au-dessus de 20°C. De plus, à même concentration, le risque de corrosion double tous les 10°C supplémentaires.
Principe de corrosion
- L'agent corrosif (par exemple les chlorures), amorce de petites piqûres dans la ‘‘couche passive’’ de l'inox.
- Un couplage galvanique s'établit entre :
• La zone de piqûre qui n'est plus passive et qui devient ‘‘zone anodique’’
• La zone restée saine, qui est passive et donc ‘‘zone cathodique’’.
La différence de potentiel des 2 zones entraîne une forte densité du courant anodique depuis le point de piqûre. Ce courant génère une forte acidification qui elle-même augmente la densité du courant de dissolution.
Le phénomène de corrosion ainsi amorcé est auto-entretenu et amplifié.
Afin de débarrasser l'intérieur de la cuve de toute particule étrangère, décontaminer et passiver l'inox, il sera effectué un traitement de nettoyage/passivation comme ci-après :
• Lavage soude caustique concentration 2 % à 65°C durant 15 minutes
• Rinçage à l'eau claire, température ambiante
• Lavage passivation avec acide nitrique à 15/20 %, à température ambiante durant 30 minutes
• Rinçage final à l'eau claire, température ambiante.

В «инертном» состоянии они покрыты защитной прослойкой окиси хрома, невидимой и «непроницаемой» для воздуха, с высокой химической стойкостью, называемой инертным слоем.
Данный слой самопроизвольно «зарубцовывается» на воздухе, т.е. он слабо разрушается, тем не менее нержавеющая сталь не является полностью ненарушимой.
Она может быть нарушена при соприкосновении с «коррозионными» веществами или при механических воздействиях (удары, царапины, попадание частиц железа во время сварочных работ, шлифовка, резка обычной стали вблизи…).
Основные коррозионные вещества - это ионы галогенов (хлориды - флориды - йодиды - бромиды) и продукты переработки серы.
Их предельную концентрацию трудно установить, поскольку на процессы коррозии оказывают влияние многие параметры (см. следующую страницу).
В качестве примера, изготовители стали считают, что существует риск коррозии нержавеющей стали 316L - Стандарт 1.4404, при контакте с раствором, содержащим более 150 ч./млн. хлоридов, при температуре выше 20 °C. Более того, при одинаковой концентрации через каждые дополнительные 10 °C риск коррозии удваивается.
Принцип действия коррозии
- Коррозионное вещество (например, хлориды) начинает маленькие точечные отверстия в «инертном слое» нержавеющей стали.
- Гальваническая связь устанавливается между:
• зоной точечного отверстия, которая более не является инертной и становится «анодной зоной»;
• ненарушенной зоной, которая является инертной и, таким образом, «катодной зоной».
Разница потенциалов 2 зон приводит к высокой плотности анодного тока начиная от точечного отверстия. Этот ток создаёт сильное окисление, которое, в свою очередь, увеличивает плотность тока электрохимического растворения.
Начатый таким образом коррозионный процесс самоподдерживается и увеличивается.
Для освобождения внутренней полости резервуара от всех частиц примесей, обеззараживания и пассивации нержавеющей стали, выполняются работы по чистке/пассивации, как указано ниже:
• промывка каустической содой - концентрация 2 % при температуре 65 °C в течение 15 минут;
• ополаскивание чистой водой при комнатной температуре;
• пассивация – промывка 15/20 % раствором азотной кислоты комнатной температуры в течение 30 минут;
• ополаскивание чистой водой комнатной температуры в конце.

L'idée d'uNopia m'est apparue comme évidente quand je cherchais à faire conjuguer ensemble des envies profondes et apparemment contradictoires : un désir intense de liberté par le voyage, ma passion pour l'instrument le plus sédentaire du monde (après l'orgue !) : le piano et l’envie de moderniser l'univers de la musique dite « classique ».
Sans faire preuve de démagogie, en reconnaissant donc le fait que cette musique demande une initiation ou un coup de foudre, mais aussi en fuyant l'élitisme et les a priori de classes, je revendique l'idée de l'utopie telle que l'avait pensée Thomas More au 16ème siècle.
Son livre, Utopia, était à la fois un récit de voyage et le projet rationnel d'établissement d’une société idéale. Il s'agissait donc pour lui d'élargir le champ du possible littéraire et politique, faisons-le, en musique !
Mon expérience des concerts, des divers spectateurs et surtout des publics « oubliés » comme dans les internats pour jeunes en difficultés ou les prisons m’a montré la portée immense de cette musique que j'avais connue au biberon.
Considérant que cette musique peut toucher au cœur n'importe qui, n’importe où et à n'importe quel moment, il faut donc à présent partir, traverser les frontières, amener la musique à son public, jouer partout et tout le temps !

The idea of uNopia seemed obvious to me when I was looking to combine seemingly contradictory desires: a yearning for freedom through travel, my passion for the piano (second only to the organ when it comes to stationary instruments!), and the aspiration to modernise the world of “classical” music.
Recognising that this kind of music requires initiation or else instant attraction, but also avoiding any elitism or preconceptions of social classes, I sympathise with the idea of utopia as described by Thomas More in the sixteenth century. His book, Utopia, was at once a travel narrative and a rational project to establish an ideal society. For him it was about expanding the field of literary and political possibility. Let us do the same in music!
My experience with concerts, various audiences, and especially “forgotten” audiences like those in boarding schools for youth in difficulty or in prisons showed me the immense impact of this music that I was raised on.
Considering that this music can touch the heart of whoever, wherever and whenever, we must go, cross the borders, bring the music to its audience … To play everywhere and all the time!

• A relatively small number of people – about ten or fewer – will own the corporate stock.
• All or most of the owners will participate directly in managing and running the business; investors who don’t directly participate will generally be limited to friends or family members.
• All of the owners will live in the state in which you form your corporation and conduct your business.
A. The Structure of a Corporation
Corporations are controlled primarily by state, not federal, law. This means that 50 different sets of rules cover how corporations are created. Terminology differs from state to state. For example, most states use the term “articles of incorporation” to refer to the basic document creating the corporation, but some states (including Connecticut, Delaware, New Jersey, New York, and Oklahoma) use the term “certificate of incorporation.” Tennessee calls it a “charter,” and Vermont uses the term “articles of association.” Fortunately, the similarities in corporate procedure outweigh the differences, so most of what you find in this chapter will apply to your own situation. Nevertheless, watch out for the differences.
People involved in a corporation traditionally play different legal roles: incorporator, shareholder, director, officer, employee. We’ll look at those roles here. But, in virtually every state, there’s a way that you can set up a corporation in which one or two people play all roles.

Les différentes tuyauteries (process, nettoyage ou fluides auxiliaires) raccordées à la cuve ne doivent pas créer de contraintes sur les piquages, par leur poids ou leur dilatation. Utiliser impérativement des tuyaux flexibles ou tout autre compensateur de dilatation.
Selon sa construction et son équipement, la cuve sera raccordée aux fluides et énergies (eau, vapeur, air comprimé, électricité …) en respectant scrupuleusement les instructions spécifiques éventuelles ou le plan d’ensemble de la cuve, particulièrement au niveau des débits, pressions, températures et le cas échéant au niveau des dispositifs de sécurité dont elle doit être équipée (soupape, disque d'éclatement, vanne de barrage, vanne de décompression, …) selon les limites de fourniture prévues au contrat.
En outre, pour les cuves et les circuits de climatisation, l'utilisateur doit s'assurer, par rapport à son installation globale, que les dispositifs de sécurité sont suffisants.
Pour éviter tout risque de brûlures des personnes par les alimentations des fluides chauds (vapeur, condensats…), ces éléments devront être protégés par tout moyen approprié (protection collective ou individuelle).
Pour les branchements des fluides, l'installateur utilisera des accessoires de raccordement adaptés.
Mise à la terre (équipotentialité des masses)
Nos cuves sont équipées au niveau du châssis ou d’un pied d’une patte de mise à la terre. Il appartient au client, même si la cuve ne possède pas d’appareils électriques, d’effectuer son raccordement électrique à la masse générale de l’installation par un conducteur de protection, de forme et de dimension appropriées.
Raccordement des tuyauteries de nettoyage, process ou autres
- Si la cuve est équipée de soupapes, celles-ci seront, dans la mesure du possible, canalisées au sol pour éviter les projections sur les utilisateurs.
- Sur une cuve à entourage flottant (en plusieurs parties par recouvrement), les réseaux de tuyauteries devront être libres en dilatation.
- Les raccordements seront effectués comme ci-dessous.
- Respecter les débits et pressions des solutions de nettoyage qui sont précisés sur le plan de la cuve.
- Tous les réseaux de tuyauteries devront être libres en dilatation et ne pas générer de contraintes sur les piquages de la cuve.
Raccordement aux fluides auxiliaires, climatisation par circuit forcé
- Protection des circuits par soupapes -
- Prévoir la mise en place d'une soupape pression en entrée du circuit de climatisation pour protéger celui-ci contre toute surpression (pression irrégulière du réseau, coups de bélier, ouvertures trop brusques de vannes).
- Ajout d’accessoires sur la cuve -
En cas de présence de passerelle supportée par la cuve, consulter Pierre Guérin pour déterminer la masse admissible sur la cuve et la position du support.
- Protection contre les chocs thermiques -
Les passages successifs des fluides chauds et des fluides froids dans les circuits de climatisation génèrent des chocs thermiques qui augmentent la fatigue du métal, accélèrent le vieillissement des équipements et peuvent être la cause de sa destruction.
- Si votre cuve est équipée d'un circuit de climatisation recevant successivement plusieurs fluides (chauds et froids), sa durée de vie sera limitée à un nombre de cycles, fonction des fluides utilisés ainsi que des paramètres de chauffage et de refroidissement.
Tarer la soupape à la pression de conception précisée sur le plan d'ensemble de la cuve (3.2 bar pour une pression d’utilisation de 3 bar).
- Prévoir la mise en place d'une soupape casse-vide en sortie du circuit de climatisation pour protéger le circuit de la cuve contre toute dépression, si nécessaire, provoquée par exemple lors des opérations de process ou de nettoyage de la cuve (phases successives de chauffage et de refroidissement).
- Vannes d’entrée & de sortie des fluides auxiliaires -
- Des coups de bélier importants peuvent se produire lors des ouvertures de vannes sur les fluides.
Les coups de bélier génèrent des contraintes mécaniques pouvant détériorer les circuits de climatisation.
Prévoir la pose de vannes anti-bélier (vannes à ouverture et fermeture progressives).

Різні трубопроводи (технологічні, очисний або допоміжних рідких середовищ), приєднані до резервуара, не повинні створювати на урізаннях навантаження, пов’язане з їх вагою або розширенням. Обов’язково використовувати гнучкі шланги або будь-який інший компенсатор розширення.
Залежно від своєї конструкції та оснащення, резервуар приєднується до рідких середовищ та інженерних мереж (вода, пара, стиснуте повітря, електрика…) в строгій відповідності з робочою документацією чи креслеником загального виду резервуара, особливо щодо продуктивності, тисків, температур та, в відповідному випадку, щодо обладнань безпеки, якими резервуар повинен бути устаткований (клапан, захисна діафрагма, клапан-відсікач, клапан скидання тиску, …) згідно з об’ємом поставки, передбаченим в договорі.
Крім того, для резервуарів та системи охолодження, користувач повинен переконатися в тому, що засоби захисту достатні для його установки в цілому.
Щоб повністю усунути ризик одержання опіків при приєднанні гарячих рідких середовищ (пар, конденсати…), необхідно захистити персонал усіма належними засобами (колективного чи індивідуального захисту).
Для приєднання рідких середовищ фахівець, здійснюючий монтаж, використовує узгоджену арматуру.
Заземлення (вирівнювання потенціалів)
Наші резервуари обладнані на рамі чи ніжці скобою заземлення. Навіть якщо у резервуара немає електроапаратури, користувачу належить виконати своє електричне заземлення на корпус захисним провідником відповідної форми та розміру.
Приєднання очисних, технологічних та інших трубопроводів
- Якщо резервуар обладнаний клапанами, для запобігання випліскувань на користувачів клапани, наскільки можливо, устатковуються відводом до землі.
- В трубопроводних мережах резервуара з плаваючою покрівлею (з кількох частин нахлистом) треба передбачити компенсатор розширення.
- Приєднання треба виконувати як показано нижче.
- Дотрімуватися вказаних на плані резервуара вимог щодо витрати та тиску моючих розчинів.
- Усі трубопроводні мережі повинні мати вільне теплове розширення та не створювати напруження на урізаннях резервуара.
Приєднання допоміжних рідких середовищ, система охолодження від напорної мережи
- Захист мереж за допомогою клапанов -
- Передбачити встановлення на місці клапана скидання тиску на вході системи охолодження для її захисту від будь-якого надмірного тиску (непостійний тиск мережи, гідравлічні удари, занадто різкі відкриття клапану).
- Додання допоміжного обладнання на резервуар -
- При наяві перехідних містків, що опираються на резервуар, проконсультуйтесь з фахівцями компанії Pierre Guérin, щоб визначити припустиму для резервуара масу та розташування кріплення.
- Захист від термоударів -
- Проходження змінюючих одне одного гарячих та холодних середовищ через систему охолодження створюють термоудари, які збільшують втому металу, прискорюють процес старіння обладнання та можуть стати причиною його руйнування.
- Якщо ваш резервуар обладнаний системою охолодження, по якій проходять, змінюючи одне одного, декілька робочих середовищ (гарячих та холодних), його строк служби обмежений числом циклів, призначенням використовуваних рідких середовищ, а також параметрами нагрівання та охолодження.
Виконати тарування клапану на розрахунковий тиск, вказаний на кресленику загального виду резервуара (3,2 бар для робочого тиску 3 бар).
- Передбачити встановлення клапана зриву вакууму на виході системи охолодження для захисту, при необхідності, мережи резервуара від будь-якого зниження тиску, викликаного, наприклад, технологічними операціями чи чищенням резервуара (послідовні фази нагрівання та охолодження).
- Впускні та випускні клапани допоміжних рідких середовищ -
- При відкритті клапанів у рідких середовищах можуть створюватися значні гідравлічні удари. Гідравлічні удари створюють механічні впливи, які можуть пошкодити систему охолодження.
Передбачити розташування гідравлічних клапанів (клапанів послідовного відкриття та закриття).

Source text - Ukrainian Вагітна жінка у віці 39 років надійшла до приймального відділення з діагнозом: вагітність перша, 21 тиждень; легенева артеріальна гіпертензія ІІІ ст., асоційована з вродженою вадою серця (синдром Тауссіг-Бінга) – великий дефект міжшлуночкової перегородки (МШП), транспозиція магістральних судин; стан після паліативної операції Сеннінге (1993); недостатність тристулкового клапану ІІІ ст., недостатність мітрального клапану ІІІ ст. за ВООЗ; вторинний еритроцитоз. Дані анамнезу, дані об’єктивного огляду, показники лабораторних та інструментальних досліджень, а також вид оперативного втручання, дозволили вибрати найбільш оптимальним методом анестезії – внутрішньовенну з інтубацією трахеї та ШВЛ.
Ключові слова: вагітність, малий кесарів розтин, загальна анестезія, легенева гіпертензія, синдром Тауссіг-Бінга, серцева недостатність, клінічний випадок.
Вступ. Вада розвитку серця, відома як аномалія Тауссіга-Бінга (також відомий як синдром Тауссіга-Бінга) складається з транспозиції аорти в правий шлуночок і неправильного положення легеневої артерії з дефектом сублегеневої міжшлуночкової перегородки (ДМШП) (Мал. 1) [1]. Частота народження дітей за вадою Тауссіга-Бінга не перевищує 1% всіх вроджених вад серця. Вада частіше вражає чоловіків ніж жінок, в співвідношенні від 2:1 до 3:1 відповідно [2].
Подібні клінічні випадки зустрічаються дуже рідко. Вагітність у жінок з аномалією Тауссіга-Бінга у пізніх строках зазвичай призводять до важкої декомпенсації і, як наслідок, до летального результату. Саме тому, для запобігання подальших важких наслідків, застосовують оперативне втручання – малий кесарів розтин (при недоношеній вагітності, коли ще не утворився нижній сегмент матки). Пацієнти з легеневою гіпертензією, що підлягають хірургічному втручанню, входять в групу вираженого ризику по періопераційній захворюваності та смертності. При позасерцевій операції частота смертності досягає 7% [3]. Дихальна недостатність і недостатність правих відділів серця є найбільш частими причинами смерті.
Наводимо приклад клінічного випадку. Вагітна віком 39 років надійшла до приймального відділення з діагнозом: вагітність перша, 21 тиждень; легенева артеріальна гіпертензія ІІІ ст., асоційована з вродженою вадою серця (синдром Тауссіга-Бінга) – великий дефект міжшлуночкової перегородки (МШП), транспозиція магістральних судин; стан після паліативної операції Сеннінге (1993); недостатність тристулкового клапану ІІІ ст., недостатність мітрального клапану ІІІ ст. за ВООЗ; вторинний еритроцитоз. Об’єктивно, загальній стан середнього-важкого ступеню. Свідомість ясна, хвара доступна продуктивному контакту. Скарги на час огляду на слабкість, швидку втомлюваність, виражену задишку змішаного характеру в спокої, що збільшується під час фізичної активності, відмічає періодичні епізоди нестачі повітря, під час яких вона змушена користуватись кисневим концентратором для інсуфляції кисню через кисневу маску, чи назальні канюлі, такі епізоди частішають під час фізичної активності, або емоційного навантаження. Хвора астенічної будови тіла. Спостерігається виражений дефіцит ваги (ІМТ = 14,17 кг/м2, зріст 168 см, вага 40 кг). Візуалізується акроциоаноз та ціаннотичність слизових оболонок, набряк ніжніх кінцівок. Дихання жорстке, ослаблене в нижніх відділах легень, частота дихальніх рухів підвищена та складає 30-34 вдихи за хвилину. Екскурсія грудної клітки знижена.
Translation - English A 39 y. o. pregnant woman came to the admissions department with the following diagnosis: first pregnancy, 21 weeks; class III pulmonary arterial hypertension associated with congenital heart disease (Taussig-Bing syndrome) – severe ventricular septal defect (VSD), transposition of the great vessels; condition after palliative Senning procedure (1993); class III tricuspid valve insufficiency, class III mitral valve insufficiency per WHO; secondary erythrocytosis. Anamnesis, physical examination findings, laboratory tests and clinical investigations results, as well as a surgical intervention type were rationale to choose as the best anaesthesia method an intravenous anaesthesia with tracheal intubation and artificial lung ventilation (ALV).

Introduction. A heart malformation called Taussig-Bing anomaly (also known as Taussig-Bing syndrome) consists of transposition of the aorta to the right ventricle and malposition of the pulmonary artery with subpulmonary ventricular septal defect (VSD) (Fig. 1) [1]. The rate of Taussig-Bing defect in newborns does not exceed 1% of all congenital heart defects. This defect is more common in men than in women, in the ratio from 2:1 to 3:1 according to [2].
Such clinical cases are very rare. Pregnancy in women with Taussig-Bing anomaly usually results in severe decompensation during late pregnancy and, consequently, in fatal outcome. This is exactly why in order to prevent further severe complications, surgical intervention is employed – minor cesarean section (under incomplete pregnancy, when the lower uterine segment has not formed yet). Pulmonary hypertension patients subject to surgical intervention belong to a pronounced risk group by perioperative morbidity and mortality. The mortality rate for noncardiac surgery is as high as 7% [3]. Respiratory failure and insufficiency of right heart are the most frequent causes of death.
Here is an example of a clinical case. A 39 y. o. pregnant woman came to the admissions department with the following diagnosis: first pregnancy, 21 weeks; class III pulmonary arterial hypertension associated with congenital heart disease (Taussig-Bing syndrome) – severe ventricular septal defect (VSD), transposition of the great vessels; condition after palliative Senning procedure (1993); class III tricuspid valve insufficiency, class III mitral valve insufficiency per WHO; secondary erythrocytosis. Objectively, the overall medical condition is moderate. The patient is alert and open to productive communication. As of the time of the examination, the patient complains of weakness, easy fatigability, pronounced mixed dyspnea at rest that increases during physical activities; the patient reports periodic episodes of stifling from want of air, during which she has to use an oxygen concentrator for oxygen insufflation through an oxygen mask or nasal canals; such episodes become more frequent during physical activities or emotional strain. The patient is of asthenic body type. Significant weight deficiency is observed (BMI = 14,17 kg/m2, height 168 cm, weight 40 kg). Visible are acrocyanosis and cyanoticity of mucous membranes, edema of lower extremities. Strident breathing, diminished in lower lungs; respiratory rate is elevated and equals 30-34 breaths per minute. Reduced chest excursion.

I started learning French at a ripe old age of 6 years old and “a little” later was a secretary of the USSR-France Friendship Society. I earned a Master’s Degree in Mathematics and Education with a minor in Computer Science from Odessa State University, Ukraine, and worked as a teacher of mathematics and a computer programmer. I moved to the US in 1991 and, since 1992, have been a full time translator/interpreter, member of the American Translators Association, ATA-certified from English into Russian. From 1995 to 2007, I worked full time as an over-the-phone interpreter for Language Line Services (top category interpreter, certified for interpreting in medical, legal, insurance, and finance fields). Completed a medical terminology college course, "Bridging the Gap" medical interpreter training program, and Design and Interpretation of Clinical Trials course by Johns Hopkins University. From 2008 to 2020, I worked full time as an in-house technical and legal translator/interpreter/technical writer for Sakhalin-1 project where I translated thousands of pages of engineering and procurement documents, contracts, and AutoCAD drawings. Published a short story in a literary magazine in the US (in English). In 2023, I moved to France. On a personal note, my interests include guitar music and knitting.
