I, as patient, authorize my admission to FHASC, Inc. for outpatient care and treatment. I consent to the administration of all routine and emergency treatment, procedures and medications, which the physician(s) responsible for diagnosis and treatment feels are necessary or desirable. FHASC, Inc. maintains personnel and facilities to assist with various surgical operations and other special diagnostic and therapeutic procedures. These surgical operations and special diagnostic or therapeutic procedures all involve RISKS OF COMPLICATIONS, SERIOUS INJURY OR EVEN DEATH, from both known and unknown causes. I understand that, except in emergency or extraordinary circumstances, consent will be obtained from me or on my behalf before administration of procedures, and that my physician will discuss such procedures with me or the person consenting on my behalf prior to obtaining such consent.
Statement of Financial Liability: I understand that I am responsible for the full payment of the bill and the fact that I have assigned insurance benefits does not relieve my obligation to pay the bill in total. Unless other payment arrangements have been made in writing, any outstanding balance will be due within 30 days. I understand that FHASC, Inc. is not responsible for items lost or stolen.

I am a Spanish Interpreter,Translator and Spanish/English Teacher based in Lima Peru. I started in Lima translating from English to Spanish and I had all kind of texts, from Psychology to Airplanes, and then in Colorado, USA, I translated texts for companies from technology,medical science, law to animal rights.
I started my Interpreter carreer in Colorado Springs, USA wotking for several Interpreting companies there, then in Lima I worked in several events and clients too.
As a Spanish and English Teacher I taught hundreds of students in the US and Peru either in language schools or private online and in person classes.
