Here's the context: Cáncer de próstata en retención: es necesaria la RTU simultánea a la hormonoterapia para el alivio de la obstrucción.
The only term that I am having trouble with is 'en retención'. Retaining and withholding just don't fit here. Any suggestions?

This is referring to the urinary retention associated with/caused by prostate cancer and so the prostate cancer itself is not 'in retention'. It is in fact the patient, or the bladder, that is in retetion. In your case the 'in' is misleading. With slightly more context you could use 'retention associated with prostate cancer' or 'urinary retention associated with prostate cancer'.
