Is this sentence grammatically correct in the original Croatian?

"Ovaj obrazac predstavlja informaciju za ispitanika i obrazac za informirani pristanak na farmakogenomsko istraživanje i dodatak je obrascu za informirani pristanak koji ste već potpisali za kliničko ispitivanje ####."

I'm pretty sure it means:

"This form presents information for the study subject, an informed consent form for pharmacogenomic research, and an annex to the informed consent form that you have already signed for clinical trial ####."

However, the original doesn't seem right to me, particularly the way "dodatak je obrascu" is worded. Is the "je" necessary here?

it is grammatically correct and "je" is necessary because there are two clauses: 1) Obrazac predstavlja...., 2) obrazac je dodatak....

It is a new form which is an addendum to the already signed.

...i dodatak je obrascu za informirani pristanak koji ste vec potpisali ...

translates into

..."and it is an addendum [the new form] to the informed consent form that you have alread

if you take "je" out then one "i" goes out also

Thank you Veronica. I do realize that the patients have already signed a consent for another part of the study. What is confusing me here is the grammer. Are "informaciju" "obrazac" and "dodatak" all objects of the verb "predstavlja"? If so, why is there a "je" between "dodatak" and "obrascu"?? Thank you, Eric
BTW, I do agree with you that "addendum to informed consent" is a better translation that "annex to informed consent", which is what I had written. However, what is confusing me here is the wording of the entire sentence. Thank you, Eric
