Toilets
I would understand it to be the number of "facilities" on each floor, not the number of toilets per facility.
I think I would just say "Toilets per floor" (who'd have thought loos could be so difficult...)

4 restroom cores / 4 bathroom cores
From what the few Google hits for "WC-Kern(e)" tell me, it refers to facilities rather than individual stalls or commodes.
Quite a few hits for restroom core(s) and bathroom core(s) in architectural contexts, so this seems a pretty safe bet to me.

: yes, to 'core', like 'core' for elevator shaft since the plumbing is arranged in the same area on each floor

(4) WC units
WC means Water Closet (although nobody asks "where's the water closet"). People call it toilet but an architect or estate agent (who deals in property) would use the term WC. The word "core" doesn't work here prefer "unit".
