Too late, but ... I've already delivered the translation and my solution was: "Transcript of registration of death (Supplementary/2010)". So, no points. Not knowing the details of the Belgian system, I decided to use a formulation that would apply whether it was a transcription of an entry in a register or of a certificate. Though "supplétoire" could be translated as "suppletory", I thought that was a bit archaic.

My British birth certificate is headed:
"Certified copy of an entry of birth".
I suggest substituting death for birth.

Certified copy of an entry of death is the official title of the document in the UK. Suppletory is a legal term meaning a court ruling to supply the item.

Thanks Claude-Andrew, however see my discussion note. Your second reference is to a certified copy of a certified copy of a 19th century register, probably because the original register is held locally and the copy at the General Register Office was the one used.
