This is all I have. It's from an ophthalmological examination for someone with recurrent uveitis.

fccwl This phrase most likely reads this in Polish: "V oc. sin. = 0,9 cc wł.", which means "Visual acuity in the left eye = 0.9 with the patient wearing their own spectacles" (cc wł. = cum correctione + własne)
