This is a check box item in a patient medical history form, same as the last one, pre-op/consultation questionnaire. This check box is under the category of " do you have allergies? If so, which/to what?" Since this appears with penicillin, latex/rubber, etc. I am wondering if it is something as simple as plaster for a cast of some sort. But still, not too sure.
